1. Has the individual done EXTRA duty to benefit the Association? e.g. Served as an elected officer, held a committee chair position or served on a committee.
2. Has the individual been an ACTIVE member and attended meetings and group functions? HAIP has a membership meeting/function each month.
3. Has the individual participated on a State, Regional or National Level? (List Activities)
4. Has the individual completed educational courses and earned a designation? (List Courses & Designation)
5. Has the individual been involved with non-NAIW industry Association and/or Community Affairs? (List details)
6. Has not been a past recipient of this award or current President of HAIP.
